GRADEセンター：設立のための最小限の基準

国内において、GRADEに関する理解を向上させるためには、GRADEセンター（あるいは、GRADEネットワーク）の設立が必要です。センターの役割として、定期的なWSの開催を考える必要があり、海外からの講師招聘も出てくるでしょう。つまり資金や人材が必要となってきます。

海外では、コクラングループの下にGRADEセンターを作成しているところもありますが、日本での設立となると、現実的には（研究費の有効活用ということから）Mindsが最適と思います。

しかしその場合、Mindsは診療ガイドライン作成手法として、２つのシステム（MindsシステムとGRADEシステム）を提示することになり、診療ガイドラインの作成者・利用者に大きな混乱をもたらすことになります。

現在のMindsマニュアル・手引き2014は、GRADEのほとんどを利用した内容ですが、いくつかの重要ステップがGRADEとは異なっています。
したがって、Mindsは、”現在のMindsマニュアルはGRADEシステムを改変して作成したものである”と宣言し、GRADEの基本に準じた手法に一本化するための改訂が必要と思います。
また、”Minds2007手引きは使用しないことを強く勧める。Minds2007手引きを利用して作成した診療ガイドラインは早急に改訂すべきである”という勧告も必要です。

さて、現時点で、GRADEセンター（GRADEネットワーク）が海外にどれほど存在しているのかを、Holger Schunemann教授に伺った結果が以下の８個です。

でもって、GRADEセンター設立のための最小限の基準が以下です。

Minimum criteria for establishing GRADE centers

The mission of a GRADE center would be:
  a) To teach and train in the use of GRADE
  b) To support those requiring support
  c) To contribute to the further development of GRADE

1. Adequate involvement in GRADE

a)	Center leaders should have attended GRADE meetings.

2. Adequate training methods

a)	Candidate GRADE centers need to ensure that they have adequate capacity to perform GRADE tasks. This can be achieved by having experienced GRADE members spend time at the site and help training others.
b)	GRADE centers need to ensure that they are up to date with GRADE methods. Attendance at GRADE meetings, obtaining updates are critical.

3. Adequate capacity

a)	Candidate GRADE centers need to have a minimum number of staff (suggested 3) who dedicate a large part of their time to GRADE activities

4. Adequate dedication to training

a)	Candidate GRADE centers should provide or contribute to workshops to train in GRADE methods.

5. Adequate financial support and sustainability

a)	Candidate GRADE centers need to have dedicated funding (direct or indirect) to sustain function for at least 5 years on approval. Institutional support (not financial) of the activity should be provided..
