メタアナリシス:異なる尺度を使った連続変数の効果サイズの5つの表示法

研究の最後に1度だけ測定される2値アウトカムと違い、連続変数は研究の開始(ベースライン、つまり観察開始前または介入実施前)と終了の両方の時点で測定されます。したがって、連続アウトカムは研究のベースラインから終了時にかけてのスコアの変化(すなわち、変化スコア)または研究の終了時における最終測定値(すなわち、最終値)として表現することができます。この場合、想定リスク(スコア)とは、対照群における最終値または変化スコアのことを指します。連続アウトカムのための2つの重要な統合効果推定値が、平均差(MD)または標準化平均差(SMD)です。

システマティックレビューにおいては、しばしば異なる尺度を使った測定法により連続変数の研究を扱う場合があります。複数の研究において、同一の構成要素の測定に異なるツールが使われている場合は、連続データを統合するためのメタアナリシスにおいては、SMDを利用することがよいとされています。

しかし、JCE GRADEシリーズにおいては、連続アウトカムに関するbody of evidenceの質評価の際(特に異なる尺度を使った連続変数の効果サイズを扱う際)、5つの表示法を解説しています。

MIDが不明の研究が組み入れられていても、MID既知の他の研究のSDを参考にして、統合解析処理が可能というその後の報告(Johnston BC. et al: JCE 2012; 65 817-26)もあります。

これらの5つのアプローチは、システマティックレビューや診療ガイドラインにおける連続アウトカムに関する効果サイズの精確さを考える上で重要ですが、現状のGRADEpro/GDTはすべてに対応してはいません。
